BBL in Turkey: safety, recovery and what to ask before booking

A Brazilian butt lift (BBL) uses liposuction to take fat from the abdomen, flanks or back and injects it into the buttocks. It has carried a higher risk of fatal fat embolism than most cosmetic operations, which is why international safety guidance says fat must be injected only above the gluteal muscle, never into it. Plan 10–14 days in Istanbul and several weeks of limited sitting.

What it is

A BBL has two parts: liposuction to harvest fat and shape the waist, then purification and injection of the fat into the buttocks. Part of the transferred fat does not survive, so volume decreases over the first months. The main danger is fat entering the large veins of the buttock muscle and travelling to the lungs (pulmonary fat embolism). In response, plastic surgery societies recommend subcutaneous-only injection, ideally guided by ultrasound.

Techniques

Subcutaneous fat grafting

Fat is placed only under the skin, above the muscle – the safety standard recommended by plastic surgery societies.

Ultrasound-guided BBL

Real-time ultrasound lets the surgeon see the cannula stays above the muscle.

“Skinny BBL” / small-volume

Less fat transferred, for patients with limited donor fat.

Buttock implants

An alternative when there is not enough fat; a different operation with its own risks.

Who it suits

Usually a good candidate

  • Enough fat to harvest from the abdomen, flanks or back
  • Stable weight and good general health
  • Willing to follow sitting restrictions for several weeks

Think twice / discuss first

  • Very little donor fat
  • You plan to lose a lot of weight
  • Smoking, clotting disorders or heart/lung problems
  • Any clinic that cannot explain how it keeps injections above the muscle

Your stay in Istanbul, step by step

  1. Day 1Consultation, markings, tests.
  2. Day 2Surgery (2–4 hours) and usually one night in hospital.
  3. Days 3–10Hotel recovery; lie on your front or side, sit only on a BBL cushion if allowed, walk regularly.
  4. Days 10–14Final check; the surgeon decides when you can fly and how to manage a long seated journey.

Recovery timeline

  1. Weeks 1–3No direct sitting on the buttocks (or only with a special cushion); compression garment on the liposuction areas.
  2. Weeks 2–3Desk work possible with sitting precautions.
  3. Week 6Usually cleared for exercise.
  4. Months 3–6Part of the fat volume is lost; the shape stabilises by about 6–12 months.

Risks and complications

  • Fat embolism – rare but potentially fatal; the reason for the subcutaneous-only rule
  • Blood clots, especially with long flights
  • Fat necrosis, lumps, asymmetry, loss of volume
  • Infection, seroma
  • Liposuction risks at the donor sites

This list is not exhaustive. Your surgeon should explain the risks that apply to you, in writing, before you consent.

Cost: what drives the price

Price depends on the liposuction areas, the volume transferred, whether ultrasound guidance is used, the hospital stay and what the package includes (garments, cushion, check-ups, hotel). A very low price is a reason to ask more questions about safety.

Questions to ask the clinic

  • Do you inject fat only above the gluteal muscle? Do you use ultrasound guidance?
  • How many BBLs does the surgeon perform, and who does the injection itself?
  • Which areas are liposuctioned, and how much fat will be transferred?
  • What blood-clot prevention do you use, and how should I handle the flight home?
  • Who looks after me if there is a complication after I return?

Frequently asked questions

Is a BBL in Turkey safe?

The risk depends mainly on technique and team, not the country. Choose a qualified plastic surgeon in a licensed hospital who injects fat only above the muscle, ideally under ultrasound guidance.

How long can I not sit after a BBL?

Surgeons commonly recommend avoiding direct sitting for 2–3 weeks or using a special cushion. Follow your surgeon’s instructions.

How long should I stay in Istanbul after a BBL?

Usually 10–14 days, so that early check-ups happen before a long seated flight.

Will all the fat stay?

No. Part of the transferred fat is absorbed in the first months; the result stabilises after about 6 months.
